Perform evaluations and treatments as needed while ensuring the highest degree of quality care is maintained.  Write clear, realistic, measurable goals specific to the individual patient needs.  Treat patients with respect, dignity, and privacy.  Motivate patients to participate in therapy treatment.  Use creative therapy interventions specific to the patients’ interests and needs to obtain positive patient outcomes and progress. Participate as an interdisciplinary team member to determine potential patient needs and plan of care.  Accurately complete documentation of qualitative and quantitative data of patient progress within designated time period.  Demonstrate the ability to utilize the computer system effectively, adhering to company policy.

Crowne Healthcare of Eufaula is a 180-bed Skilled Nursing Facility in Eufaula, AL (The Bass Fishing Capital of the World!) - one hour south of Columbus, GA and one hour north of Dothan, AL. They've recently completed a remodel to add more private rooms, a complete rehab wing with a lounge complete with a Wi-Fi sitting area. They'll be starting another remodeling project to add a Transitional Care Unit that will include a fully functioning kitchen, bathroom and laundry area. The Therapy Department has two gyms with one having a wall of windows overlooking an outside area that is also used for therapy activities in the warmer months. Crowne of Eufaula is also a Dementia Focus Facility with a separate unit for Alzheimer/Dementia patients. From this unit they have access to an outdoor setting area and a private gazebo. They also have a dedicated staff specially trained to work with Alzheimer/Dementia residents with their own Activities Director and CNAs to care for these patients. They treat a large number of highly functioning residents with roughly 60% of their caseload returning to home or assisted living. The Administrator, Director of Nursing and the entire Nursing Staff are very "Pro" therapy. They have a great working relationship with the therapists and help to keep each other updated and informed on the residents.
