Perform evaluations and treatments as needed while ensuring the highest degree of quality care is maintained.  Write clear, realistic, measurable goals specific to the individual patient needs.  Treat patients with respect, dignity, and privacy.  Motivate patients to participate in therapy treatment.  Use creative therapy interventions specific to the patients’ interests and needs to obtain positive patient outcomes and progress. Participate as an interdisciplinary team member to determine potential patient needs and plan of care.  Accurately complete documentation of qualitative and quantitative data of patient progress within designated time period.  Demonstrate the ability to utilize the computer system effectively, adhering to company policy.

This is a 130 bed Skilled Nursing Facility and Rehab Center located in Crystal River, FL. We offer a wonderful team, full of knowledge and compassion who work very well together. Communication is a must with daily stand up meetings and weekly rehab meetings to discuss any pertinent information regarding our pts. We have a very low turnover in staffing. We offer 6-7 day a week therapy with an available prn pool to assist in staffing needs. he facility is the oldest in the county however, has been beautifully updated. It offers an excellent Activities Program with outside entertainers for the enjoyment of our residents. The therapy gym is shared by OT/PT offering a small kitchen area, parallel bars, many exercise machines, pain modality equipment, mat table, stairs and ability to order any needed equipment with an excellent view of the grounds. ST has their own room with the privacy they need to conduct their treatments. The administration and nursing staff are very supportive of therapy and work quite well together. We have a nice mix of patients. We have higher functioning patients who stay with us for a short time and return to home or ALF. We also have our long term patients who are like a part of our family that we also enjoy treating to improve their quality of life. We have several programs in place, including: Pain Management, Wound Management, Falls Management, Hearing, Dysphagia and Dementia Management, Functional Decline, and Step Forward.
